OSHA Approves New Reporting Forms

The federal Occupational Safety and Health Administration has updated three of its record-keeping forms, to be officially implemented on January 1, 2002. OSHA's avowed purpose is simplification and clarification, as well as to provide a clearer indication of whether an illness or injury actually is work-related; for instance, the new forms provide new definitions of medical treatment, first aid, and restricted work. The forms are:

Form 300 - Log of Work-Related Injuries and Illnesses (simplified and printed on standard letter-size paper).

Form 300A - Summary of Work-Related Injuries and Illnesses (a separate form updated to make it easier to calculate incidence rates).

Form 301 - Injury and Illness Incident Report (includes more data about how the injury or illness occurred).

Although the forms won't be valid until next year, there may be some point to using the coming eight months to obtain copies and become familiar with them. For more information, and to download forms, visit www.osha-slc.gov/recordkeeping/index.html.
